There is a playbook, and once you see it you cannot unsee it. A charismatic voice tells you your fatigue and your brain fog might be your genes. You buy a test. Someone you have never met calls to read you a result you do not understand, and builds you a stack of "methylated everything" that runs into the hundreds a month. Here is the part that gets left out: the gene they built an empire on does not say what they tell you it says. And the actual research, the kind with citations rather than testimonials, points somewhere much less glamorous and much less expensive.
First, what is actually real -- because this part matters
Before taking anything apart, be clear about what is not in dispute, because a critique that throws out the biology along with the marketing is just the same mistake pointing the other way. Methylation is real. One-carbon metabolism is a genuine, well-mapped set of biochemical reactions that your body runs constantly to build DNA, process amino acids and regulate gene expression. MTHFR is a real enzyme with a real job in that pathway, and the common 677C>T variant genuinely does reduce how well it works. Folate, B12 and homocysteine are all measurable in ordinary blood work. Riboflavin-derived cofactors really are required at multiple steps of mitochondrial one-carbon metabolism, and severe riboflavin deficiency really is linked to anaemia. None of that is the problem. The problem is the distance between that biochemistry and what gets sold on the back of it.
The test at the centre of it cannot do what it implies
Start with the product that opens the funnel: the consumer genetic report. A systematic review of 118 studies found internet-based direct-to-consumer genetic tests were generally not informative, with little predictive power, and did not measure genetic risk appropriately. That is not a critic's take, it is the literature. And it gets sharper. In a database of 1.7 million people, 41 percent carried at least one genetic variant nobody could interpret, a so-called variant of uncertain significance. When those uncertain variants were eventually sorted out, about 80 percent turned out to be benign, meaning they meant nothing at all. In a separate cohort of 1.45 million people, of the variants that got reclassified, 91 percent were downgraded to harmless. Surveyed consumers routinely misread their own reports, confusing "you have a variant" with "you are at risk". Now look at what you pay. Flagship methylation gene tests in this space run around 599 dollars and read a couple of dozen hand-picked genes. For 99 to 199 dollars, a mainstream ancestry-and-health kit reads hundreds of thousands to a million positions across your whole genome. True whole-genome sequencing, reading all 3.2 billion letters, has been sold for roughly 170 to 500 dollars. So the premium panel charges you more to look at dramatically less. Our view, stated as our own and not as a citation: this is the tell. The expensive test and the cheap test have the same interpretation problem, so the price was never buying better science. It was buying a smaller test with a bigger story attached. When someone hands you a genetic printout and a supplement invoice in the same breath, the printout is the sales tool.
MTHFR: the most marketed gene, and what the research says
MTHFR is the star of the show, so let us be precise, because precision is exactly what the hype skips. Our library holds hundreds of studies touching MTHFR. Not one of them shows that having an MTHFR variant means you need methylated B12. The research is real, and it is about folate, the nutrient the enzyme actually helps process. Then there is the part the funnel would rather you did not read. The American College of Medical Genetics and Genomics, the professional body that credentials this kind of testing, published a practice guideline whose title is simply "lack of evidence for MTHFR polymorphism testing", first in 2013 and reaffirmed by addendum in 2020. The American College of Obstetricians and Gynecologists reached the same position. The people who actually set the standards for genetic testing looked at MTHFR and concluded it should not routinely be ordered, while an entire consumer industry was built on ordering exactly that and selling the fix. Here is the finding that belongs on the flyer instead of the gene. In 6,076 adults, carrying the higher-risk MTHFR genotype together with low riboflavin status carried roughly a three-fold higher blood-pressure risk, while the variant on its own carried about 42 percent. Read that again. What made the gene matter was not a methylated megadose. It was riboflavin, vitamin B2, an ordinary cofactor most people have never been told to think about. Our view: that is the whole story in one study. A gene does not act alone, it acts inside a crew of cofactors. The marketing takes a real gene, attaches it to the wrong nutrient, sells the expensive form of that wrong nutrient, and skips the cheap one the data actually points at. Real science, wrong conclusion, premium price.
- The honest limit on that headline finding: it is an observational cohort, not a randomised trial of giving riboflavin. It shows the variant's consequence tracked with riboflavin status, which is a strong argument for covering cofactors and not proof that a B2 tablet lowers your blood pressure.

"You need methylated everything" -- where it falls apart
The pitch is that ordinary vitamins are useless and you need the active, methylated form of everything. Two problems. One: for B12, the ordinary route already works. A systematic review of randomised trials found oral B12 raised blood B12 as effectively as an injection, with neurological improvement on both, and a primary-care trial normalised B12 in over 90 percent of people at eight weeks by mouth. Your gut is not the broken middleman the pitch needs it to be. Zinc absorption, for one, climbs to around 92 percent efficiency when your diet runs low, because the body adapts to need. The mouth is the route the body is built for. A bypass is a tool for a specific case, not a way of life. Two: more is not free. Piling in high doses of single active nutrients has documented downsides. Excess folic acid can correct the anaemia of a B12 deficiency while masking and potentially worsening the nerve damage underneath. Selenium's benefit follows a U-shaped curve, helpful when you start low and no better, or worse, when you pour more on. There is a documented case of real human harm from too much zinc, taken correctly, for too long, and zinc is a nutrient we point people toward. Our view, and we will say the uncomfortable part out loud because it is true even about things we could sell you: too much of a single nutrient does not make a cell healthier, it overwhelms it. Methyl forms can genuinely help pull someone out of a critical state, and that is real. Living on high-dose methylated single nutrients indefinitely is a different claim, and nobody has the evidence for it.
"But it is liposomal, it absorbs better" -- the honest version
This one deserves a straight answer, because the honest version is more damning than the lazy one. Liposomal vitamin C does get more into your blood. Reviews show higher peak blood levels and one crossover trial saw a several-fold jump. So "liposomal does not absorb" would be false, and we are not going to tell you that. Here is what is missing. Not one study shows that the higher blood level made anyone healthier. A bigger number on a blood draw is not a health outcome. And the gut gatekeeping that fancy delivery bypasses exists on purpose: the body regulates how much of many nutrients it lets in, and pushing more past that gate is not automatically a benefit. Our view: "it absorbs better" is a lab-number claim dressed up as a health claim. Getting more into your blood is the easy part to prove and the easy part to sell. Whether it did anything good for you is the hard part, and that is the part the receipt never mentions.
Follow the incentive: the test, the call, the stack
None of this is an accident. It is a business model, and it is worth naming the shape of it without naming anyone, because you already recognise it when you see it. The test creates a problem. Then an advisor gets you on the phone, and in these programmes that is typically a commissioned sales role paid on what it sells rather than on whether you get better. The job listings say so out loud: base plus uncapped commission. They turn your result into a monthly protocol. Reporting on these funnels describes initial spends of 500 to 1,000 dollars before supplements even start, then a recurring stack on top. And the branded methylated multivitamin at the centre of it is the same thing as a generic methylated B-complex at 20 to 30 dollars a month, wrapped in a genetic story and marked up. The whole machine is coated in genuine, decent free advice, because that is what makes it work. Our view: the tell is not whether someone shares good information. Plenty of them do. The tell is whether the thing being sold carries the same evidence as the free advice it leads with. When the free part is "eat real food, get sunlight, manage stress" and the paid part is a thousand-dollar stack backed by a testimonial, those two halves are not being held to the same standard. That gap is the problem, and it can live inside a person who also tells you true things.
The fix that is not sexy, and has the most science behind it
Here is why the honest version is a harder sell. It is boring and it is cheap. Cover the whole crew, do not megadose one piece. A broad multivitamin and mineral foundation is the unglamorous move, and the evidence is real and honestly mixed, which is exactly how we will give it to you. In a randomised, double-blind, placebo-controlled trial, a multivitamin and mineral significantly raised a measured antioxidant micronutrient status score against placebo. In 10,698 adults in NHANES, multivitamin use was linked to fewer micronutrient adequacy gaps, which is the "fill the gaps modern food leaves" claim actually measured. The COSMOS trials linked a daily multivitamin to modest cognition benefits over three years. And now the nulls, because leaving them out is the exact thing this page is criticising. Large reviews found no overall multivitamin benefit for most healthy people on hard outcomes, and one COSMOS analysis found no reduction in cognitive impairment or dementia. The honest synthesis, plainly: a broad foundation is worth most when it is correcting a real, measured gap, rather than as blanket insurance. That is less exciting than "your genes demand this bottle", and it is what the data supports. Our view, and we will talk you out of something we could sell you: trace lithium as a nutrient is a genuinely interesting question, but lithium orotate as a magic delivery form has no demonstrated carrier mechanism, and the kidney-function signal behind the caution is an animal finding rather than a human one. Measure the right thing, the right way
If you are going to test, test something that reflects what is actually landing in your cells. Your genes do not change, and mostly cannot tell you what to do next week. You do not need methylated everything. You do not need a 599-dollar gene report read to you by a commissioned salesperson. You do not need a thousand-dollar stack built on a test the geneticists themselves say not to run routinely. You need the boring part the marketing skips: cover your cofactor crew, do not megadose one piece, measure what actually changes, and demand that the thing being sold carries the same evidence as the free advice around it. That last line is the whole point. Anyone can share good information as bait. Ask the harder question every time: where is the study?
